Chicago Fire Department Still & Box And A EMS Plan 2 3939 N. Pine Grove ( Electrical Vault In Basement ) 02/09/08 : Eight people, including one Chicago firefighter, were hospitalized Saturday afternoon when a fire broke out in a North Side high-rise building. A faulty electrical vault in the basement is being blamed for sparking the fire.

A fire broke out at 3930 N. Pine Grove Ave. in a 31-story high-rise building, according to Fire Media Affairs spokesman Richard Rosado.

The fire was initially reported around 12:30 p.m.

An EMS Plan 2, with 10 ambulances, was called about 12:55 p.m., according to Fire Media Affairs Director Larry Langford.

The fire appears to have been caused by a faulty electrical vault in the basement. Smoke from the basement fire sent smoke throughout the building, Langford said.

Although the fire had been extinguished as of 1:10 p.m., crews were initially dealing with moderate smoke conditions, Langford said. As of 2:30 all smoke had cleared out of the building.

Eight people, including one Chicago firefighter, were hospitalized in good condition, Rosado said.

Two of the people transported were asthmatic, and two were taken in for smoke inhalation. The firefighter was sent to be checked out, Langford said.

Crews have been sent to check on each individual apartment in the building to make sure people are safe, Langford said. No additional people are expected to be taken to the hospital.

There appeared to be a problem with customers' electrical equipment, and ComEd has been asked to de-energize their equipment in order to figure out what caused the fire, according to ComEd spokeswoman Arlana Johnson.

The electrical vault belonged to the building and was not ComEd equipment, Johnson said.

Due to the investigation, customers had their power shut down while a building electrician made repairs to the building's electrical vault, Johnson said.

Power has not been restored as of 8 p.m., and there is no estimate of a restoration time yet, Johnson said.

A ComEd crew and fire crews remain at the scene of the fire as of 8 p.m.

An American Red Cross spokeswoman said most residents found alternate housing for the night, which is predicted to be frigid, and their services were not needed.

CHICAGO (CBS) ¯ Fire crews are searching a Northwest Side dollar store Friday night after ceiling tiles fell prompting a still-and-box alarm. 

About 9 p.m., crews responded to the Portage Park Family Dollar at 4828 W. Irving Park Road to reports of a ceiling collapse, Fire Media Affairs spokesman Richard Rosado said. 

According to police, employees heard a loud rumbling noise -- they evacuated the store -- and minutes later, the ceiling came crashing down. 

There was no structural damage to the building, but some roofing materials fell in the store, Rosado said. No injuries were reported. 

Further details were not immediately available at 9:45 p.m.

  Chicago firefighters were battling a heavy fire in a retail/apartment building on the West Side late Wednesday. No injuries were immediately reported.

The blaze broke out before 10:30 p.m. in the building in the 4100 block of West Madison Street in the West Garfield Park neighborhood. The building contains several stores at the street level with apartments on the second and third floors, Fire Chief Kevin MacGregor said.

The fire began on the west end of the roughly 120-foot-long structure and moved to two connected buildings just to the east, MacGregor said.




"With the ice and conditions we're working in it makes it more difficult, also there was some entry problems with iron gates when we got there," MacGregor said. He said firefighters, however, were not having problems with water freezing despite the single-digit temperatures.

No injuries have been reported and MacGregor said firefighters are trying to confirm a report about a man who evacuated the building and said no one else was inside. Firefighters were waiting until after the blaze is extinguished before searching the building.

It was not clear late Wednesday how many of the apartments in the building had been inhabited, firefighters said.

Willie Patterson, who was at a nearby restaurant, said the building housed several businesses that included a barbershop, a hair salon and a small grocery.

Chicago Fire Department Level 2 Haz-Mat On 47th Street East of Western 01/05/08 : The scene of a hydrochloric acid leak from an outdoor tank that prompted a Level II HazMat Saturday morning on the Southwest Side has been secured.

The Level I HazMat was called for 4435 S. Western Ave. at around 10:30 a.m., according to Fire Media Affairs Chief Kevin MacGregor. It was upgraded to a Level II HazMat about 11:20 a.m., Fire Media Affairs Director Larry Langford said.

At the Wheatland Tube Company, 2300 W. 47th St., an outdoor vertical storage tank began leaking hydrochloric acid. The cold temperatures made the spill evident because it made the vapors visible, Langford said. 

As a precaution, the Home Depot at 4555 S. Western Ave. was evacuated because it’s "a few feet from the tank." However, Langford said, the spill is not creating a dangerous situation as of 11:30 a.m.

"To the east is a rail yard and a big parkway is to the west; so though it’s in the middle of the city, it’s really isolated," Langford said.

The spill was contained and sealed, and firefighters kept water lines on the fumes released from the acid to prevent it from spreading. Nobody has been injured in the incident, Langford said.

Chicago Fire Department Pin In Response With A EMS Plan 3 And A Level 1 Haz-Mat For The Semi Crash In China Town El Station On Cermack : 21 injured in crash, including driver  Team CoverageCHICAGO (WLS) -- A semi trailer has crashed into the CTA station at Cermak-Chinatown, killing at least two women. 
Fifteen ambulances were sent to the scene after the Friday rush hour accident. Eleven people were critically injured, said Chicago Fire Department spokesman Raymond Orozco. He also said eight people were in stable condition; two were in good condition. 

A total of four people were extricated from the scene near the station stairs and escalator area, where the truck cab was embedded. The truck driver was one of those extricated and was treated at Stroger Hospital, Orozco said. The driver then gave himself up to police. 
Two hours after the crash, Red Line service resumed for all stops, with trains bypassing the Cermak station. The stop will be closed until further notice. For status updates, click here. Story continues belowAdvertisement
 
Authorities stretched tarps around the base of the semi, indicating that there were likely victims underneath it. Two women, pedestrians, were pronounced dead at the scene, said fire department spokesman Larry Langford. 
One witness said that the semi came off the Stevenson Expressway driving fast, as if it had no brakes, and ran into the stairwell and escalator where people go up onto the train. He said several people were standing at the bus stop in the truck's path and going in and out of the train station. 
The truck crashed into the CTA stop at 138 W. Cermak Rd. about 5:20 p.m., according to police News Affairs Officer JoAnn Taylor. 
The exit to the highway comes to a T in front of the station, where drivers should make a right or left turn. The truck driver apparently did neither. 
Several people were initially reported pinned under the semi. The Chicago Fire Department said at least five children were among those taken to the hospital. Two of the children were initially reported in serious to critical condition. 
Another witness reported that the cab of the truck was propped in an upward position and said the escalator bank actually stopped the momentum of the crash after the truck rolled right through the nearby bus shelter, which was crushed up against the side of that escalator bank. 
Rescuers wirked diligently to get into the cab of the truck. The smell of diesel was reported near the crash scene. Crews first concentrated on stabilizing the semi to keep it from potentially falling over. 
The cause of the crash is still unknown. The driver is being questioned. 
The driver, 51, passed a blood-alcohol test, but would not submit to urine test. It's unknown whether he's cooperating with police. 
Witnesses say he idn't sound his horn to warn people. 
Crews brought in a wrecker and have separated the truck from the CTA station. The semi sat in the center of Cermak Rd., a dozen yards from the original scene. The truck is listed as being owned by Xtra Leasing out of St. Louis. 
CTA President Ron Huberman said he does not know when the station and will reopen. 
Huberman said there is extensive damage to the station, the escalators have collapsed, but the overhead structure is sound. The Office of Emergency Management Communication said it brought the Illinois Department of Transportation in to check the structural integrity.

Survivors Speak 
Some witnesses saw the driver before the impact. 
"He come off the expressway at a kind of fast speed. He just kept going. He was bent on the steering wheel like he had a heart attack or went to sleep," said Daryl Holbert. 
Then the impact. 
"I heard this horrible, loud crash and crunching noise. I got up and thought the El train had come off the tracks," said Michael McClure. 
But it was the semi truck - hitting one victim after another. 
"People were flying everywhere, a lot of people hurt. They were waiting on the bus, coming down the escalator. You don't expect a big semi to come at you," said Robert Moore. 
Some victims were conscious, sitting up and talking. 
"I saw maybe 10-12 victims who just had scrapes and bruises,didn't look very serious, a couple of cuts on the neck and shoulder," said Carl Flenner. 
Others were trapped and, in some cases, not moving. 
"I thought they were all dead. When the firemen and policemen arrived and started speaking to them, two of them started moving. I said, 'Oh they are alive! They are alive!'" said Dolly Turner. 
There were survivors, people who missed the accident simply because of timing. 
"I feel so blessed actually. I feel like I have a guardian angel looking over me. I just-- It's a miracle. I obviously feel bad for the people," said Carly Kaput. 
Conditions of the Injured 
The 21 injured were taken to about a half-dozen area hospitals. The E.R. doctor at Comers Children's Hospital said he was surprised that the injuries weren't more severe. The injuries to the patients at Comers appear to be non-life threatening. 
Stroger hospital received four patients, two adults and two children. A spokesperson for the hospital says all four were in stable condition. 
"We have four victims here, and I've been told two victims at Provident Hospital. They all appear to be in stable condition. I think the injuries are considered non-life threatening," said Sean Howard, Stroger Hospital spokesperson. 
Northwestern Memorial Hospital received two adults, including what appeared to be a CTA worker. The two women at Northwestern Hospital were listed in fair condition. Comer Children's Hospital at the University of Chicago Medical Center received three children - one 14 years old, one 11 years old, and the age of the third was unknown. 
"So far we've seen injuries that are at least, at this time, deemed to be not life threatening. We do have the bumps and bruises; we have cuts and lacerations," said Dr. Nicholas Strane, Comer Children's Hospital. 
Strane said he treated the children and explained the importance of having pediatric specialists available. 
"To call on the resources of a trauma level one center, which has surgery and neurosurgery if needed," he said. 
The doctors were waiting for test results on the three children at Comers, but if they appear to be Ok, they were likely to be released Friday night.
